What DPS Needs for testing:
1.DE964 Form (green certificate from Alliance)
2.TEA Form/VOE from the attendance office at your high school or GED/Diploma
3. Original U.S. Birth Certificate
Passport with USA Visa
Green Card
(Only One of the above is needed)
4. Original Social Security Card- no photocopies
5. DL14 Form filled out in black ink and signed by Parent/Legal Guardian
6. $16.00
*Alliance Driving School also offers a testing service where we will take the student down to DPS to get their permit processed and photo taken for a small additional fee.
